Chatham Hills Subacute Care Center, a leader in comprehensive healthcare services, is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled team. In our state-of-the-art facility, we prioritize exceptional patient outcomes and a compassionate approach to healthcare. The selected candidate will play a pivotal role in delivering tailored nursing care to patients undergoing short-term rehab, receiving subacute care, and those in long-term care settings. Administer nursing care to ill, injured, or disabled patients Monitor patient health and record signs Administer medications and provide proper documentation Provide basic patient care and treatment Collaborate with registered nurses to administer prescribed medications Educate patients and their families on health conditions and preventive care Ensure compliance with all health care regulations and maintain patient confidentiality Proficient in administering medications Strong organizational and communication skills Ability to handle physical workload Detail-oriented and able to multitask effectively Medical Insurance Dental Insurance Vision Insurance Current LPN license in the state of employment CPR certification
