Anticipated procedures/ surgeries: Vaginal/ Cesarean deliveries, D&C, Laparoscopy/ Laparotomy Description of Work Assignment Provider will cover Labor and Delivery 2 days per week Breakdown of Clinic Days/Sessions and/or OR Days 24 hours per week. 2 12 hour sessions Schedule of Days/Times Needed to Work/ Shift Hours: Monday 7pm-7am, Thursday 7pm-7am On call? No Patient Load 3-5 Deliveries per shift 0-1 Cesarean per shift Board Status Requirement (BC/BE) BC Is there additional support staff? No Is the provider working solo or with other providers? If so, whom? 3 Residents/ 1 Midwife Are there Advanced Practice Providers? If yes, is the locums provider responsible for them? Residents and Midwives Trauma Level of Work Location No EMR System? EPIC Is the Clinic EMR Different from the Hospital EMR?
