MUST BE A CERTIFIED HOME HEALTH AIDE
CHHA REQUIREMENTS:
- 2 forms of identification (one must be an ID and the social security card)
- A valid CHHA license
-Proof of immunity for Measles, Mumps and Rubella
- 1 Step PPD test within 1 year or Chest X-Ray (valid for 5 years)
