Job Description:
Heart to Heart Home Care - Visits residential programs on a
routine basis and as specific needs arise. RN will also be
responsible to perform start of care and home visits for in home
cases as needed.
Requirements:Must be at least 18 years of age or olderMust have a
High School Diploma or EquivalentMust pass a Criminal Background
CheckLicensed Registered Nurse will not be listed on the Central
RegistryGraduate of approved school of Practical NursingLicensed
and currently registered in the State of New Jersey with a current
valid license in good standing as an RN issued by the NJ Board of
NursingMinimum of 2 years' experience as a Registered Nurse
Skills:Organizational skills and attention to detailStrong oral and
written communication skills and interpersonal skillsCreativity and
problem solving1 year of experience working with individuals with
developmental disabilities preferredDemonstrates ability to lead
and teach effectively
Training:Must complete and show proficiency with mandatory DDD
trainings with the appropriate timeframe, including:Overview of
Developmental DisabilitiesAbuse and NeglectMedication
AdministrationCPR and First AidAgency OrientationAgency's approved
training on handling life-threatening emergencies/Danielle's LawAll
additional trainings as necessary
Essential Job Functions:Provides oversight and support for all
medical and healthcare related needs of consumers. Conducts
training for management and community support staff.
Develops and maintains an interactive relationship with each
consumer that reflects values and principles held by Heart to Heart
Home Care.
Provides nursing services to consumers by identifying and reporting
on health needs of consumers through observation of their physical,
social and emotional needs and through review of their medical
records.
Administers nursing care to consumers as needed according to the
care plan.
Contributes to the development, implementation and evaluation of
care plans through conferences with the group home manager, case
manager, guardian/family and medical professionals.
Participates in IHP/IDT meetings. May assist in the development of
the IHP.
Coordinates service provision within the community in conjunction
with the Group Home Manager/DDD Case Manager in order to meet the
individual medical needs of residents.
Attends medical appointments and transports consumers to community
destinations as necessary or assigned in Heart to Heart's vehicle.
Schedules medical appointments when appropriate.
Exhibits proficiency and implements all tasks needed to service the
individuals appropriately from a medical perspective.
Reviews all medical related documents and medication
packs/containers to ensure proper administration and
documentation.
Administers medication and documents in compliance with Heart to
Heart Home Care's policies and procedures which conform to DDD
guidelines.
Orients new community support staff to the medical needs of the
specific consumers with whom they will be working.
Ensures that policies and procedures that guide and support the
provision of care are implemented and the recommendations to
improve performance of care are implemented within Heart to
Heart.
Reviews and maintains the system of clinical records, reporting,
reporting forms and service data, ensuring that all documentation
is timely, appropriate, accurate and within agency guidelines.
Identifies clinical problems and works out effective action plans;
communicates unresolved problems to supervisor.
Monitors and evaluates the quality of care provided to
consumers.
Performs on-call duties.
Performs all applicable duties of a registered nurse and ensures
that professional standards of practice are maintained.
Must cooperate with the licensee and Department of Human Services
staff in any inspection or evaluation.
